    Causes

    The optic nerve can be damaged directly, from a penetrating injury, or indirectly, when the force of a blow to the head or forehead is transmitted through the bone surrounding the nerve. Fractures of the orbit or skull base are common associated findings.

    Symptoms

    Sudden vision loss following head or facial trauma is the primary symptom, ranging from mild blurring to complete blindness in the affected eye. A relative afferent pupillary defect (sluggish pupil response) is often present on examination.

    Diagnosis

    CT imaging of the orbit and skull base is used to assess for fractures and nerve compression. A detailed eye examination, including pupil testing and visual field assessment, helps determine the extent of the damage once the patient is medically stable.
