Traumatic optic neuropathy is a sudden and often severe impairment of the optic nerve resulting from direct or indirect physical injury to the head or face. It occurs when the delicate nerve fibres are crushed, sheared, or compressed following a high-impact event, disrupting the transmission of visual signals from the eye to the brain. Because rapid, irreversible cellular death can follow the initial injury, prompt medical evaluation is essential to determine the likely course of visual recovery.

Causes
The optic nerve can be damaged directly, from a penetrating injury, or indirectly, when the force of a blow to the head or forehead is transmitted through the bone surrounding the nerve. Fractures of the orbit or skull base are common associated findings.
Symptoms
Sudden vision loss following head or facial trauma is the primary symptom, ranging from mild blurring to complete blindness in the affected eye. A relative afferent pupillary defect (sluggish pupil response) is often present on examination.
Diagnosis
CT imaging of the orbit and skull base is used to assess for fractures and nerve compression. A detailed eye examination, including pupil testing and visual field assessment, helps determine the extent of the damage once the patient is medically stable.
Treatment — Stem Cell Therapy
Stem cell therapy is studied for its potential to support optic nerve tissue affected by traumatic injury, once the acute injury has fully stabilized. It is not appropriate during the acute emergency phase and is only considered after the treating trauma team confirms stability.
