    Causes

    Wolfram Syndrome is caused by mutations in the WFS1 gene (or less commonly WFS2), which affects cell function across multiple organ systems including the pancreas, optic nerve, and inner ear.

    Symptoms

    Optic atrophy is typically the second feature to appear (after diabetes mellitus), usually in the first decade of life, causing gradual vision loss. Hearing loss and diabetes insipidus often develop later.

    Diagnosis

    Genetic testing for the WFS1 gene confirms the diagnosis. OCT and visual field testing assess the optic nerve component, while endocrinology and audiology evaluations assess the other associated features.
